1. Please fill out the form below on your computer and agree to the terms in the Healers Waiver.
*Registration as a SF Dragon Healers is not complete until all forms are filled out and submitted. You will not be allowed to practice until the Healers demographic form and CDBA forms are filled out and submitted.

2. NEW HEALERS: Click on the link below and enter the team login information. (Returning Healers: click here to log in using the e-mail registered from previous years and skip to step 5)
3. Click "Submit"to enter new member login information on the next page.
4. Continue to the Registration Login Page to create a new profile. Emergency contacts MUST be somone who is NOT on the team.
5. After creating a new profile, click "Renew Membership" to review your profile information.
6. Click "Continue to Waiver" to complete the online Waiver.
